Reporting Guidelines for Health Professionals and Mandated Reporters

How Do Health Care Professions Report Communicable Disease On-Line?

Doctors and other health care providers are required to report a number of communicable diseases to local health departments. Now you can electronically report infectious diseases to the state and local health departments through the Illinois National Electronic Disease Surveillance System (I-NEDSS). Once you learn the process of reporting online, the process is quick and efficient. Online reporting will enhance our ability to respond to potential health emergencies. For I-NEDSS training you may call the Illinois Department of Public Health: 217-782-2016

What do I do if my patient works in a sensitive occupation (food handler, daycare provider, healthcare worker)?

Please report to the Communicable Disease Investigator at (217) 531-2932 and please tell your patient the following: see link

When health care workers, food handlers or people who work in day care centers or nursing homes are diagnosed with Shigella, Salmonellosis and E. coli they must get follow-up tests to confirm that they are free of the disease before going back to work. These tests are free to those who have no insurance coverage for the tests. Contact the Public Health Communicable Disease Investigator at 217-531-2932 for test kits and information.

What do I do if my patient has been exposed to Zika Virus?

Communicable Diseases

Communicable Diseases

Public Health Departments are required to investigate communicable (infectious) diseases such as whooping cough, tuberculosis, hepatitis and foodborne or waterborne illnesses. Our Communicable Disease Investigator works with the state health department and local health care providers to identify, investigate, and try to control disease outbreaks in our community.

Any person having knowledge of a known or suspected communicable disease is required to report the information to the Champaign-Urbana Public Health Department. Information from the Illinois Department of Health on Infectious disease reporting.

Complete this online form or call the Communicable Disease Investigator at 217-531-2932.

What do I do about Animal Bites?

Rabies is a preventable viral disease of mammals, most often transmitted through the bite of a rabid animal. The vast majority of rabies cases reported to the Centers for Disease Control and Prevention (CDC) each year occur in wild animals like raccoons, skunks, bats, and foxes. Seek medical care for any animal bite. Was there an exposure? See our rabies exposure algorithm. Get more information about rabies.

What do I do about head or body lice?

Head lice infest the head and neck and attach their eggs to the base of the hair shaft. Lice move by crawling; they cannot hop or fly. Head lice are spread most commonly by close person-to-person contact. Both over-the-counter and prescription medications are available for treatment of head lice infestations. More information about lice.

What should I do if I have bed bugs?

Bed bugs, a problem worldwide, are resurging, causing property loss, expense, and inconvenience. The good news is that bed bugs do not transmit disease. The best way to prevent bed bugs is regular inspection for signs of an infestation. More information about bed bugs.

CUPHD offers bed bug identification. If you are unsure if what you are seeing is a bed bug, you may bring one in for examination. Please call 217-531-2932 if you are interested in bringing a bug for identification.

As the Emergency Support Function (ESF) #8 lead for Champaign County, the Champaign-Urbana Public Health District works closely with local, state and federal partners to restore communities to pre-incident conditions and to lessen the effects of public health emergencies, whether they be natural, technical or man-made.

The list of services goes on and on. Each of the Health District divisions - Administration; Community Health Nursing & Clinical Services; Community Health Surveillance, Planning & Education; Dental Health; Environmental Health; Family Health; and HIV/STD/TB Prevention & Management - serves a unique public health function. All programs within these divisions are designed to address health priorities in the Champaign-Urbana community.

Health District offices are located in Champaign, Urbana and Rantoul, but we also bring our services to numerous other sites in Champaign County.

We have a contractual relationship with the Champaign County Public Health Department Board of Health, which provides funding to support services to ensure food and water protection, safe sewage, and investigation and mitigation of communicable diseases to county residents outside Champaign and Urbana.

We also partner with dozens of other health and social service providers, sharing resources and expertise to get health services to the people who most need them.
