H1N1 Vaccine Walk-In Clinics Scheduled

The Champaign-Urbana Public Health District will have Walk-In Clinics this Week for Priority-Age Children

Champaign, IL – The Champaign-Urbana Public Health District (CUPHD) has received enough H1N1 vaccine to hold walk-in clinics for a top priority group this week.  Only children from the ages of 6 months to 5 years will be able to receive the H1N1 vaccination.

The walk-in clinics will be held at CUPHD (201 W. Kenyon Road in Champaign) from Tuesday, October 20th to Friday, October 23rd.  The clinics will run from 10:00 AM to 7:00 PM each day and there is no cost for the vaccine.  Clinics will be cancelled if the vaccine runs out prior to the last scheduled day.

For faster service, please print and complete consent forms prior to arriving at CUPHD.  You can find the forms at http://www.c-uphd.org/documents/h1n1/H1N1-consent-schoolen.pdf.  Forms must be signed by a parent or legal guardian.
